Delmarva Cat Connection, Inc. (herein referred to as DCC) is placing __________________ (herein referred to as the cat)   up for adoption for a fee of ________. The cat is a rescue cat whose background and medical history may be unknown to DCC. Therefore, DCC cannot guarantee the temperament, habits, or health of the cat. The Adopter understands that it is the responsibility of the Adopter to evaluate the cat before agreeing to adopt the cat and sign this contract. Upon signing this contract, the Adopter accepts full financial and legal responsibility for this cat.

The Adopter agrees to abide by these “Terms of Adoption” listed below:

Care of the Cat: The Adopter agrees that the cat is being adopted as a family pet only and will be an INDOOR cat only. The Adopter agrees to provide the cat with fresh water, wholesome food, love, and attention. The Adopter agrees that the cat will be properly restrained in a secure crate whenever transported in a vehicle.

Veterinary Care: The Adopter agrees to have the cat regularly examined by a licensed veterinarian to maintain it in good health.  The Adopter agrees to keep rabies, distemper, and other vaccinations current. In the event of illness or injury, the Adopter agrees to seek prompt veterinary care. The Adopter may not euthanize the cat except in the case of terminal illness, injury, or old age accompanied by pain and suffering. In these cases, the euthanasia must be recommended and performed by a licensed veterinarian.

Sterilization: If any kitten is adopted prior to sterilization, it will be the responsibility of the Adopter to have the kitten sterilized by the age of 6 months. The Adopter will be responsible for sending a copy of the sterilization certificate to DCC no later than 2 weeks after the operation. It will be considered a breach of contract if the kitten has not been sterilized by 6 months of age and no certificate showing proof of sterilization has been received by DCC. Failure to comply with this sterilization request will result in DCC reclaiming ownership of the kitten.___________ ADOPTERS INITIALS  

Return Policy: If for any reason, the adoption is not satisfactory to the Adopter and the Adopter decides to relinquish the cat, the Adopter agrees to return the cat to DCC. The adopter agrees to give DCC sufficient time to secure a proper facility to board the cat. The Adopter will continue to provide a safe and loving environment for the cat until it is returned. All medical information must be placed in the cat’s folder and returned with the cat. There will be no refund after 14 days from the date of adoption. ______________ADOPTERS INITIALS

Rights of Inspection: DCC reserves the right to follow up on this adoption in order to protect the welfare of the cat. The Adopter agrees to give DCC access to the cat and the Adopter’s premises with 24 hour notice, either verbally or in writing, to check on the physical, mental, and living conditions of the cat.     

Right to Transfer: If at any time in the future the Adopter cannot continue to provide proper care for the cat, the Adopter will not abandon the cat or give or sell the cat to another company, organization, animal shelter, or medical research organization. The cat cannot be transferred or sold to any other person without prior approval from DCC.

Breach of Contract: DCC reserves the right to terminate this contract if the Adopter misrepresents any information provided in the application agreement or if any of the above “Terms of Adoption” are not met to the specifications of DCC. These infractions will be considered a breach of contract at which time DCC reserves the right to reclaim the cat.

Release & Indemnification: The Adopter hereby releases DCC, its Board of Directors, and/or any employees or volunteers from any and all liability for personal injury, property damage, legal fees, veterinary care, or any other expense or liability incurred by the Adopter as a result of this adoption. The Adopter agrees to indemnify DCC from any and all claims and to pay, without limitation, any costs related to such injury, damage, or liability, including, in case of litigation, any attorney fees incurred by DCC in its defense.  

I certify that I am at least 21 years of age and that I have the consent of the other members in my household to bring a cat into our home. If I rent my home, I agree to provide a copy of my lease stating that I have the right to own a cat.
